RN Care Manager

Our client has an immediate opening for two RN Care Managers – one in the areas north of Boston and one south of Boston. This role will guide, coach, and connect caregivers to the resources they need through in-person home visits and virtual support.

Responsibilities:

• Conduct home visits within assigned territory to assess safety, gather health updates, and provide clinical coaching
• Support caregivers through a digital collaboration app by offering coaching, resource referrals, and guidance based on care plans
• Build trusting relationships with caregivers to help reduce burnout and promote well-being
• Document visits and communications promptly, ensuring regulatory compliance
• Collaborate with internal teams and participate in projects as needed

Requirements:

• Active RN license in the state of employment
• At least 2 years of recent experience working directly with older adults or individuals with disabilities
• Strong background in care planning, assessment, and case management
• Comfort with using digital tools, case management systems, and Microsoft Office
• Valid driver’s license, reliable transportation, and proof of insurance
